Please feel free to print out the forms, fill them in, read, sign and date the Fee Policy, Authorization for Disclosure, and Notice of Privacy Practices in the appropriate spaces and bring them to your initial visit.

Most insurances are accepted.  Please contatct your insurance carrier to determine your benefits.

We will need a copy of your driver’s license in order to validate your identity.
We will need a copy of your insurance card in order to process your information.
Please bring all of the above to your first appointment.

Cancellation
If you are unable to keep an appointment, you are kindly asked to provide at least a 24 hours notice.  This advance notice will allow appointments to be offered to other clients.  A fee may be charged if the patient does not show up for an appointment.

Advantages of Private Pay

Clients often consider the option to not use their insurance.

By paying privately:

  • You are free to decide what therapist you want to see.  Third-party payers will not be making the decision of which therapist you see. 
  • You have maximum privacy.  Most insurance companies require a diagnosis code as part of the billing process.  Some insurance require specific details regarding clients’ concerns and treatment plans and this can become a part of your permanent medical record.  Some insurance companies have a joined group insurance plan and the insured may have signed away the rights to confidentiality.
